What documents do I need to provide as proof of my household's expenses?
You must provide the following documents to prove any expenses that you
claim on your application:
Child and adult dependent care expenses
- Cancelled checks or receipts from childcare provider
- Letter from provider stating fees charged for services and frequency of services
Heating, telephone and electricity costs
- Bills or statements from utility companies
Rent or mortgage payments (proof must include the amount paid and a signature)
- Cancelled checks or receipts for rent paid
- Mortgage statement
Medical expenses (only medical expenses not covered by an insurance program)
- Bill or invoice from doctor, hospital or clinic
