How do I prove my identity?
The following documents serve as proof of identity. You will often need to present two of these documents to prove identity for particulat benefits:
- Driver's license/TN State Non-Driver Identification Card
- Passport
- Birth Certificate
- Social Security Card
- Baptismal/Other religious certificate
- Official school records (for children)
- Adoption records
- Official Hospital/Doctor birth records
- Naturalization certificate
- Marriage records
