What documents do I need to prove my household's income?
For each of the following types of income, provide one of the following documents:
Earned Income (wages and salary)
- Recent paycheck stubs (One month of your most recent pay stubs, regardless of how often you get paid)
- Letter from employer on company letterhead, signed and dated
- Income tax return
- Business records
Self-employment Earnings
- Signed and dated income tax return and all schedules
- Records of earnings and expenses
- A Notarized letter from a client or other business associate can help
Unemployment Benefits
- Award letter/Certificate
- Benefit check
- Correspondence from New York State Department of Labor
Private Pensions/Annuities
- Statement from pension/annuity
Social Security Benefits
- Award letter/Certificate
- Benefit check
- Correspondence from Social Security Administration
Child Support/Alimony
- Letter from person providing support
- Letter from court
- Child support/alimony check stub
Worker's Compensation
Veteran's Benefits
- Award letter

Military Pay
Interest/Dividends/Royalties
