What is it?
The MEMPHIS Plan is a healthcare ministry of the Church Health Center that assists low- income, uninsured workers in accessing affordable healthcare through a network of volunteer physicians and donated medical services. The MEMPHIS Plan is not insurance. Employers can offer the MEMPHIS Plan to those employees who are currently uninsured, earn less than 200% of federal poverty and work 20 or more paid hours weekly. What do I get? The MEMPHIS Plan provides access to primary and specialty care physicians, limited hospitalization, lab and diagnostic testing, dental services and prescription discount cards. How long do I get it? It is available to eligible employees (between the ages of 18 and 65) whose employer has agreed to make the MEMPHIS Plan available. As long as the employer is willing to participate, eligible employees can participate in the services. |

| The cost of the the MEMPHIS Plan is $45 per employee, per month and the employer agrees to pay $10 of each employee's monthly fees. Uninsured dependents can be enrolled for an additional $25 per dependent per month; with $120 per month being the maximum a family would pay monthly. Doctor visits cost $5 per visit and the dental services cost $10 per visit. | ||
